We offer a holistic and client-centered approach to understanding the needs of our older adult clients and provide solutions to challenging situations in times of uncertainty.

Compassionate Care for Life

Phone Consultation

Speak with our team to provide an overview of the client's situation so that we can determine whether we can provide beneficial guidance and advocacy.

In-Person Consultation

2 hour minimum appointment. Meet with our team to discuss the unique situation, challenges, and needs in order to develop a personalized strategy and plan for aging.

Care Assessment

4 hour minimum appointment to develop a Care Plan detailing the next steps, resources, and outcomes that will give decision-makers the tools for navigating the established path or the option to foster ongoing Care Management services from ECM.

Care Management

Ongoing Care (based on an hourly rate). You will be working with our Care Managers on an ongoing and fluid basis to implement the provided Care Plan and to help families navigate the changing dynamics of the individual’s care needs.
